As a plastic surgeon in Jaipur, one of the most common questions I receive about dimpleplasty is, “Where exactly should the dimple be placed?” Many patients are surprised to learn that there isn’t one single “correct” spot. A dimple that looks charming on one person might look unnatural on another.

The true “ideal” position is not a fixed point on a map but a personalized location that harmonizes with your unique facial anatomy. Simply copying a placement can lead to an artificial look. In this post, I’ll explain how we move beyond outdated, generic methods to find the perfect, natural-looking placement for your dimple creation surgery.

The Myth of the “One-Size-Fits-All” Dimple

For decades, many surgeons relied on a single, standardized guide known as the Khoo Boo-Chai (KBC) point. This spot is found by drawing two imaginary lines:

  • A vertical line down from the outer corner of the eye.
  • A horizontal line across from the corner of the mouth.

The point where they cross was considered the “gold standard” for dimple placement. However, modern research has shown a critical flaw in this method: a majority of naturally occurring dimples are not located on this exact point.

A one-size-fits-all approach ignores the most important factor: your individual face shape.

Why Your Face Shape Determines Your Dimple’s Location

The goal of dimpleplasty is to create a feature that looks like you were born with it. To do this, we must match the dimple’s shape and position to your underlying facial form.

Modern analysis shows that natural dimples vary significantly based on facial structure:

  • Long and Narrow Faces (Leptoprosopic): Individuals with this face shape tend to have dimples that are also long and narrow. Their ideal placement is often right at the traditional KBC point.
  • Short and Broad Faces (Euryprosopic): People with broader faces are more likely to have short, circular dimples. The most natural placement for them is often slightly above and behind the KBC point.
  • Average Faces (Mesoprosopic): For those with an average facial shape, dimples can vary, but the ideal spot is frequently found just in front of (anterior to) the KBC point.

This evidence-based approach is far superior to a generic mark. It tailors the surgery to your anatomy, which is the key to a natural result.

Dynamic vs. Static: The True Mark of a Natural Dimple

Beyond location, the behavior of the dimple is critical.

  • Dynamic Dimples: This is the goal. A natural dimple is a “dynamic” feature, meaning it is visible only when you smile or make an expression. It disappears when your face is at rest.
  • Static Dimples: A dimple that is visible even when you are not smiling is called “static.”

When we surgically create a dimple, it will be static for the first few weeks. This is a normal part of the healing process. The true artistry of the procedure is ensuring that as it heals, it relaxes and transforms into a dynamic dimple. This evolution from a static to a dynamic dimple is the most important sign of a successful, natural-looking procedure.

My Process: Finding Your Perfect Dimple Together

Determining your ideal dimple placement is a collaborative process that combines surgical expertise with your personal goals. Here is the multi-factorial model I use in my consultations:

  1. Patient Goal Setting: We start with you. You will sit in front of a mirror and show me the location you have in mind. We will discuss your desired depth, shape, and whether you want one dimple or two. This is when we explore all customization options and dimple types.
  2. Anatomical Analysis: I will then assess your facial structure—whether it’s long, broad, or average—and analyze your underlying facial muscles. This advanced assessment requires deep anatomical knowledge to ensure the placement is not only beautiful but safe.
  3. Landmark Triangulation: I don’t rely on just one method. I will use the KBC point as a starting reference, apply the modern adjustments based on your face shape, and may also use functional tests (like asking you to suck in your cheeks) to identify the natural point of depression.
  4. The Final Mark: Together, these data points create a “zone of ideality.” We will then select the final, precise spot within that zone that you are happiest with. This marking becomes the blueprint for your surgery and is a key part of the procedural logistics we plan together.

Your Most Natural Result

Ultimately, the “ideal” dimple is one that looks like it has always belonged to your smile. It’s a feature designed in harmony with your unique face, not a stamp placed in a generic location.

If you are considering dimpleplasty, the most important step is a consultation to see if you are an ideal candidate for this personalized procedure.
