The larger the gland the more time it will take to operate.

You want to know how long you will need to dedicate to the surgery on the day of the procedure.

If the procedure is to be done under General anesthesia you may need to spend the day at the hospital so block two days from your calendar. But if the procedure is to be done under local anesthesia then keeping a day will suffice.

The procedure takes around 2 hours but traveling, pre-op preparation and post op recovery also take time that needs to be accounted for.

To Arrange Transportation

Knowing the duration helps coordinate pick-up and drop-off times with friends or family.

To Manage Work or Personal Obligations

You might need to schedule time off from work or adjust personal commitments because its not only the duration of surgery but the duration od recovery as well that you need to consider. Healing takes time.

It may see reasonable that A longer duration might indicate a more complicated procedure. But this may not always be true. Irrespective of the time it takes the complexity of a gynecomastia surgery remains more or less the same.

Some times it takes longer because access to the gland tissue from a small infra areolar incision may slow things down.

Before we delve into the specifics of how long gynecomastia surgery typically takes, it’s crucial to understand the factors that influence surgical duration. These include your age, weight fluctuations, skin elasticity, and the severity of gynecomastia, including any excess skin or breast ptosis (sagging).

I spend a considerable amount of time discussing these aspects with you during consultations to ensure we set realistic expectations for both outcomes and recovery.

When considering gynecomastia surgery, several factors play a significant role in determining the complexity and duration of your procedure:

  • आयु: As you age, skin elasticity tends to decrease. This can affect how much tissue needs to be removed and how much additional support is needed for optimal results.
  • Weight Fluctuations: If you have experienced significant weight gain followed by loss, this may lead to excess skin that requires removal alongside glandular tissue.
  • Skin Elasticity: Your skin’s ability to retract after surgery can greatly impact the final outcome. Inelastic skin might require more extensive procedures and longer recovery times.
  • Severity of Gynecomastia: The extent of breast tissue present will influence how long your surgery takes. More severe cases with additional complications like excess skin or ptosis (sagging) may necessitate a longer surgical time to achieve the best results.

During our consultations, I also discuss potential risks and complications associated with gynecomastia surgery. These include infection, bleeding, scarring, asymmetry, need for further surgery, seroma (fluid collection), hematoma (blood collection), persistent skin excess, and contour deformities.

For patients with severe gynecomastia and inelastic skin due to age or inherent skin quality, I often recommend a cautious approach that may involve accepting some degree of skin excess over the inconvenience of having larger incisions.

Gynecomastia Surgery Time frame

Typically, gynecomastia surgery takes between 1 to 2 hours.

However, this time frame can vary based on individual case complexity. More extensive cases involving significant tissue removal or additional procedures like skin excision might take longer.

The procedure starts with Infiltration and liposuction, which is the initial step for all cases of gynecomastia, even those with firm sub-areolar discs (tissue beneath the nipple) also called ‘Grade 1’ gynecomastia or ‘Puffy Nipples’.

Liposuction facilitates subsequent open excisions and makes it easier to perform other necessary steps during surgery. Infiltration and completion of liposuction may take upto an hour because of the 25 minutes waiting period between infiltration and liposuction.

It may take another hour to excise the glands and then place subcutaneous sutures.

Gynecomastia Surgery Post-Op Recovery Period

After your surgery, you’ll need to take at least 5 days off work for recovery. During this time, all movements at the shoulder joints are prohibited. You would not be able to drive.

Such a drastic precaution is required because the healing between skin and the underlying muscle needs the muscle to stay relaxed.

Your initial recovery involves resting at home but not necessarily in bed. You may walk with arms by your side for 5-7 days post-surgery. That is you may use the wrists and elbow but not open your shoulders.

The use of a compression garment is essential during the first four weeks following surgery. You will typically wear this garment between 1 and 2 months, depending on how much tissue was removed and the extent of dissection under the skin. Most patients return to work after about 7 days but should avoid repetitive and strenuous exercise for at least one month post-surgery.

Gynecomastia surgery often begins with infiltration and liposuction as the primary method to remove excess fat from the chest area, improving the contour of the chest.

Liposuction alone is particularly effective for patients who have predominantly fatty tissue contributing to gynecomastia or with patients of psuedogynecomastia. i.e fatty chest that mimics gynecomastia but glandular tissue is not present.

Excisional techniques are used for severe cases with significant excess glandular tissue or skin that can’t be treated with liposuction alone.

These procedures involve making an incision around the nipple area and removing the excess breast tissue directly, followed by suturing to close the wound.

Indications for Excision:

  • When there is localized fibrous gynecomastia under the nipple.
  • In cases with significant skin redundancy or ptosis (drooping).
  • After attempting Liposuction and finding that it did not sufficiently address the dense tissue.

The choice of surgical technique in gynecomastia surgery depends on several factors including the amount of fat versus glandular tissue present, the presence of fibrous components, and the degree of skin redundancy.

Liposuction often serves as an initial step to improve chest contour, while excisional techniques are employed when necessary to address dense or fibrous tissues that require direct removal. So duration of Liposuction procedures is shorter as compared to excisional procedures because liposuction is done in either case.

Usually I don’t place drains after the surgery unless the procedure is done in a grade 4 gynecomastia or there is a preexisting condition that requires drain placement.

Drain management is a critical aspect of postoperative care. Drains are typically used to remove excess fluid from the surgical site and prevent complications such as seromas (fluid collections).

  • Drain Placement: Depending on the extent of surgery, drains may be placed in areas where significant amounts of tissue have been removed.
  • Removal Timeline: Most drains will be removed around 5 days post-surgery. However, this time frame can vary based on strict output guidelines to ensure minimal fluid collection.
  • Output Guidelines: Arbitrarily drains should remain in place until they collect less than 20 mL of fluid per day consistently. But different surgeons may have different protocols.

Long-Term Recovery Expectations:

Long-term recovery involves several important milestones and considerations:

  • Compression Garment Duration: Wearing a compression garment for up to 4–6 weeks is recommended to assist with reducing swelling, improving skin contour, and preventing complications such as seromas or hematomas.
  • Return to Normal Activities: After approximately 6–8 weeks, you can gradually return to normal physical activities. However, it’s essential to avoid repetitive motion or strenuous exercise until your surgeon gives clearance based on your healing progress.
  • Final Contour Achievement: It’s important to understand that achieving the final contour of the chest area may take up to a year or more. Some patients continue to see improvements for up to 2 years post-surgery.

Additional Tips:
